Full Job Description

You will work closely with all members of the Respiratory multi- disciplinary team, contributing to weekly MDT meetings, supporting and independently delivering both new diagnosis, review clinics, home assessments and developing nurse led clinics.

You will be liaising and working alongside other care providers internally and externally including CWP and the Community Home Oxygen team.

The Respiratory Team is looking to appoint an enthusiastic and self-motivated Registered Nurse to the Respiratory Care Team within the Urgent Care Division. This post is to assess and support patients requiring oxygen therapy on a domiciliary basis. The role offers an opportunity to work with a dynamic respiratory nurse team to improve the care and management of patients requiring oxygen, performing assessments in a complex cohort of patients with changing health care needs. This service works across the acute hospital and community setting enabling a seamless service for patients who require oxygen therapy on a domiciliary basis. You will work closely with all members of the multi-disciplinary team both in the Trust and the community setting. You should have experience in general respiratory nursing and ideally hold a diploma or recognised respiratory course or be working towards a degree. The post will consist of hospital-based outpatient clinics and home visit assessments and
ongoing care. Client interventions can be planned or on an SOS basis. Relevant training will be provided to promote autonomous working. It is essential that you can drive and have access to a car in order to carry out home visit assessments.

The Trust's services are provided from three locations: o The Countess of Chester Hospital: providing 473 general and acute beds. o Ellesmere Port Hospital: providing 60 beds as a rehabilitation, intermediate and outpatient facility. o Tarporley War Memorial Hospital: a base for community services which serve the local rural population. The Countess of Chester Hospital NHS Foundation Trust employs 5,964 staff (headcount) to provide care and treatment to a population of over 400,000 people living in Chester and West Cheshire which includes Ellesmere Port and Neston as well as the Deeside area of Flintshire. The Trust works collaboratively within the wider Cheshire and Merseyside Integrated Care System. Due to its location on the border with North Wales, the Trust also works closely with the Betsi Cadwaladr University Health Board. The Trust treats patients from England and some parts of Wales. During 2024/2025, there were approximately 643,000 patient attendances
(inpatient, A&E, outpatient and diagnostic) ranging from a simple outpatient appointment to major cancer surgery
