Full Job Description

An exciting opportunity has arisen for a Pathway Navigator for patients on Hepatology Cancer Surveillance (HCC) and you will be a central point of contact for HCC patients. This is a key hybrid role, combining administrative responsibilities with patient-facing activity.

The role includes running outpatient Fibro-scan clinics across the 3 hospital sites. A Fibro-scan is a simple non-invasive test, it is a portable machine which is used to assess the degree of scarring in the liver.

You will provide direct support to patients to maintain their engagement in surveillance pathways, ensuring they receive their surveillance scans on schedule, working closely with the Hepatology CNS and the wider Gastroenterology team.

As the central point of contact, you will be involved in maintaining effective communication with, and on behalf of, the patient, signposting to other resources and information as defined by patient need, and liaising with staff and services to ensure tests and appointments are scheduled.

A key element of the role will be to maintain and populate tracking lists for Liver Health Check clinics and to track patients for HCC surveillance; as such, you must have excellent IT and data entry skills.

Formal training and industry delivery certification will be provided for the successful candidate. Explanation of results to patients is not expected of this role.

We operate from three main hospitals-Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community healthcare premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.

FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.

WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.

All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.

Our Community Service for adults are provided in people's homes ,community centres, clinics, GP Practices, community hospitals. Our aim is to work with people to help them remain independent , improve their health and manage their conditions through high quality care , advice and support.
