Full Job Description
You will work closely with experienced Band 6 and Band 7 nurses as part of a specialist team delivering proactive liver disease screening and early intervention. The service is focused on identifying individuals at risk of advanced liver fibrosis and supporting timely diagnosis and management within the community setting, as part of the Community Liver Health Checks (CLHC) project., Support the delivery of hepatology screening clinics in community settings and from our community liver health van to include -
Fibroscan and dry blood spot sampling (DBST).
Assist in identifying individuals at risk of advanced liver fibrosis using established pathways.
Work alongside senior nurses to perform assessments and contribute to care planning.
Provide patient education on liver health, lifestyle modification, and risk factors.
Contribute to data collection, audit, and service improvement initiatives.
Liaise with multidisciplinary teams and primary care to ensure seamless patient care.
Provide cover to drive the van as required.
