Full Job Description
To provide high quality evidence based care to patients with heart failure, and be actively involved in the on-going development & delivery of the Bolton Integrated Heart Failure Specialist Nurse Service.
The post holder will:
Provide specialist advice and support to patients, their families/carers and other professionals with regard to chronic heart failure.
Deliver and support co-ordination of holistic heart failure care through delivery of an MDT in-reach service for in-patients through to discharge.
Manage patient care and adjust treatments within local and national guidelines and evidence base.
Independently manage a caseload of patients with heart failure and provide outpatient care to patients within a hospital, community clinic or within the patient's own home.
Be actively involved in the development of an integrated chronic heart failure pathway within the hospital and the community.
Lead and develop local action/management plans to meet the supportive and palliative care needs of patients with advanced heart failure.
Work closely with the multi-disciplinary team (MDT) to deliver effective management of patients with heart failure.
Exercise high levels of judgement, discretion and decision making in clinical care.
Be responsible for education and training of the MDT in the management of heart failure.
Be responsible for data collection and completion in order to meet local and national audit requirements for patients with heart failure.
Act in accordance with the NMC Code of Professional Conduct.
Have a responsibility and legal obligation to ensure that information processed for both patients and staff is kept accurate, confidential and secure in line with the Data Protection Act (1998) and the Security and Confidentiality Policy.

An exciting opportunity has arisen for an experienced and highly motivated individual to join our integrated heart failure nursing team at Bolton NHS Foundation Trust.
The heart failure service at Bolton NHS Foundation Trust is honored to be able to offer a fully integrated service for our heart failure patients operating from a shared team hub. We currently have a 37.5 hour/week heart failure specialist nurse vacancy within our team. This role is pivotal for the assessment of patients and families referred to our service and our integrated service provides a seamless pathway of care linking the hospital and the community.
The post holder will provide an integrated service for patients with a diagnosis of heart failure. This will include community clinics and providing care for people in their own homes.
The post holder will hold their own caseload of patients and be responsible for ensuring evidence based pharmacological and non- pharmacological therapies are provided to patients with heart failure.
