Full Job Description

The CSH Specialist Heart Failure team are looking for an enthusiastic Band 6 Heart Failure Nurse to join their team which provides services across the community of NW Surrey. These services include the undertaking of patient home visits and seeing patients within Outpatient settings across the locality.

This post requires an Adult Nursing qualification, together with evidence of previous experience within this specialty.

You will initially be supported by the team to undertake a comprehensive induction and then will be further supported to undertake specialist training within heart failure to develop your knowledge and skills. This will build a solid foundation for career development within the service.

Please note that Visa sponsorship is not available for this position. Candidates must possess valid UK right-to-work documentation., The Community Heart Failure Service delivers a nurse led service for heart failure patients who reside within NW Surrey.

The team deliver autonomous evidence-based patient care through direct patient contact. This involves both complex clinical assessment and the interpretation of the findings. They promote the empowerment of individual patients, offering health education, information and advice on treatment.

Working in partnership with other professionals and agencies the team ensure a seamless provision of specialist care which is responsive to the needs of the patient. Their approach is to deliver proactive care in the patient's own home or clinics, thus minimising the likelihood of patient admission to acute based services wherever possible. The team forms an integral part in the broad range of Community Services which CSH deliver across the locality, that come together to promote the delivery of care closer to the patient's home.

The care provided from the team is based within current national standards and guidelines relevant to Heart Failure.

We recognise this as a developmental role which will bring the successful candidate the experience, training and support to enhance their skills, knowledge and practice within this very important specialty.
