Full Job Description

The aim of the post is to facilitate the early identification of patients admitted to hospital with heart failure by the detection and gathering of patients already known to have heart failure or patients who are admitted with heart failure.

To provide clinical leadership, advice, support and information to staff in respect of all aspects of heart failure care to all staff groups. The role will include the development of care pathways, improved discharge planning and improvements of discharge information. The post holder will contribute and maintain links across primary and secondary care.

The post holder will be a key member of the Heart Failure Team, providing specialist-nursing care to all patients referred to the Heart Failure service, utilising advanced nursing practice skills and knowledge to enable streamlining of the care pathway.

The post holder will act as a source of expertise and knowledge for patients, relatives and health professionals.

They will be expected to liaise with the MDT to provide assessment and follow-up of patients with a confirmed diagnosis of heart failure - ensuring optimal therapy and promotion of education towards self-management. They will be expected to liaise with GPs, community services and social services to develop a multi-disciplinary approach towards improving the care and management of patients with a diagnosis of Heart Failure.

An excellent opportunity has arisen for a dynamic and enthusiastic Band 7 Heart Failure Nurse Specialist Nurses with a broad range of clinical experience to join our team. The role requires a flexible approach to work with commitment and the ability to prioritise workload in the fast-paced environment.

The role of the Band 7 Heart Failure Clinical Nurse Specialist is to provide specialist advice and guidance to clinical teams, patients and carers in the inpatient and community settings. Their role includes; direct clinical assessment, medication prescription and detailed specialist guidance for the MDT caring for a patient with confirmed heart failure.

Essential to the role is excellent written and communication skills and the ability to work both autonomously and within the broader heart failure MDT.

The ideal candidates will be Independent Prescribers who are experienced in Cardiology. However, candidates who do not meet the full personal specification would be considered for a Band 6 developmental position (moving to Band 7 on completion of specialist competencies) if they have other relevant experience that may be advantageous to this role.

Please also note this vacancy is banded dependent upon applicants experience relevant to this post. The banding of the successful applicant/s will start at Band 6 unless applicants meet the necessary threshold of skills and experience required to start on Band 7.

If recruited at Band 6 this vacancy is advertised on a fixed term contract basis for a period up to 12 months due to this being an apprenticeship/training programme. The fixed term contract status of the post holder will be reviewed in accordance with the Trust's legal obligations regarding fixed term contracts.

A learner/applicant must be a UK, EEA, or EU resident for at least the past three years to undertake an apprenticeship training programme.

Please note all existing NHS staff will only be considered on a secondment basis. This must first be agreed with your current line manager.
