Full Job Description

The healthcare support worker role within the Integrated Care Team is an integral part of the service, providing patient-centred care to the residents of Wakefield and the 5 towns area (Castleford, Featherstone, Knottingley, Normanton, Pontefract) within their preferred place of care. You will support nurses and other healthcare professional in providing care to our patients. The Team operates 24 hours a day, 7 days a week, so its important you have the flexibility to work a variety of shifts, including days, nights, weekends and bank holidays. The role also involves significant travel between patients homes and places of work. We are looking for someone who will enjoy working directly with patients in the community and is comfortable providing personal care. Please note that this role is not on the Home Office list of occupations eligible for sponsorship under the Skilled Worker route.Main duties of the job Deliver holistic, safe, and effective care in line with Trust values and behaviours. Provide personal care to Palliative Care and Rehabilitation (Rehab) patients in their own homes, including:

  • Bathing
  • Toileting
  • Dressing
  • Repositioning patients
  • Assisting with personal hygiene and appearance
