Full Job Description

Urgent Care Services offers short-term health, therapy, and care support to help service users remain at home. It treats acutely unwell individuals to prevent hospital admissions and aids hospital discharges, operating 24/7, 365 days a year. Night care includes end-of-life support, catheter care, and urgent unplanned care provided by a dedicated nursing team. Recovery at home is proven to be the best way to regain independence, with enablement and rehabilitation as priorities. KCHFT is collaborating with East Kent Hospitals University Trust to develop a virtual ward for people with acute respiratory infections or conditions like COPD. Patients discharged early will be monitored at home via remote technology, such as apps, wearables, and medical devices, with support from consultants, specialist GPs, and advanced practitioners. The specific technology is still being developed with patient and colleague input. Looking for a new challenge in Urgent Care? This exciting opportunity may be perfect for you. As a core member of the Urgent Care Service, you will deliver essential clinical and non-clinical care, supporting patients in maintaining their independence and self-care. Your role includes:

  • Following care plans and accurately recording patient interactions
  • Conducting basic observations (BP, pulse, oxygen saturations, temperature, NEWS2)
  • Providing personal care, including washing, dressing, and mobility support
  • Promptingmedication administration and managing catheter and stoma care
  • Assisting with fluid,nutrition, wound dressings, and subcutaneous injections
  • Performing venepuncture,urinalysis, and specimen management
  • Helping patients after falls using specialistequipment
  • Supporting patients with blood glucose monitoring and risk assessments forskin integrity
  • Working within a multi-professional team, you will balance varied tasks and collaborate withregistered practitioners to deliver high-quality, compassionate care. You will ensure patients feel respected and empowered by:
