Join Frimley Health NHS Foundation Trust as a Band 3 Support Worker in our Community Bridging Service! Please note you need to have a full UK Driving Licence for this role. This vacancy is for a Part Time position, working 18 hours per week. IT IS NOT A SPONSORED ROLE as it does not meet the required salary threshold. Are you passionate about making a real difference in people's lives? We're looking for caring, flexible individuals to provide high-quality support to clients in their own homes, helping with daily living activities and promoting independence. You'll work shifts between 8am and 9pm, including weekends, and play a vital role in early identification of deteriorating patients, all while developing your skills through continuous professional development. If you have experience in health and social care, excellent communication skills, and a commitment to our Trust values-committed to excellence, working together, and facing the future-we'd love to hear from you!   • To provide high quality care to clients under the indirect supervision of Team manager within the community bridging service.
  • To support clients with activities of daily living in a clients home
  • Ensure high standards are maintained in a safe environment, which promotes equality and sensitivity for all individuals
  • To participate in continuous professional development and other team activities
  • Carry out a range of duties within agreed competency skill set e.g. meal prep
  • To ensure that all patients on the caseload are safe.
