Full Job Description

  • Provide nursing skills such asadministration ofmedication, wound care, catheter care,supporting patients at end of lifeetc.
  • Provide high-quality, person-centred care to patients in their own homes.
  • Support individuals with long-term conditions tomaintaintheir independence,wellbeingand confidence.
  • Work collaboratively with registered nurses and members of the multidisciplinary team to deliver safe,effectiveand responsive care.
  • Assistinmaintaininghigh standardsof clinical practice and service delivery.
  • Develop your knowledge and skills through training,supervisionand professional development opportunities.
  • Contribute to a positive and inclusive team environment that values diversity and respects the needs of the local community.
  • Working within the local community means that visits to patients can be undertaken on foot,providingan opportunity to work closely with people in their own environment and make a meaningful difference to their day-to-day lives.

    We are looking for a Health Care Assistant who holds an NVQ Level 3 in Health and Social Care and has experience working in a hospital or community healthcare setting. Experience of working within the NHS is essential, andpreviouscommunity experience would beadvantageous.
