Sefton Community Neighbourhood Nursing have an opportunity for Band 3 Health Care Assistants to join their District Nursing Teams across the Sefton Locality

The postholder will work as part of the multidisciplinary team and will assist and support the clinical team(s). This may include working within the patient's home environment and other clinical settings within community., The post holder is responsible for the delivery of clinical care and support under the supervision of a registered nurse to a defined population. Supported by the nursing team they will deliver care within the boundaries of their role. They will work effectively and collaboratively within the team to meet the needs of patients and support the delivery of policy and procedures.

Ensure the delivery of the quality strategy within the designated area incorporating establishment of systems and processes.

The postholder must have the ability to travel independently across the Sefton Locality to deliver care and treatment to patients within their own homes.
