Full Job Description
Working closely with our Community Nursing Teams, as a Healthcare Assistant you will support and assist the senior team members to provide care to people in their own homes for a wide range of conditions including diabetes, wound and end of life care. Our work is crucial to the community as we aim to prevent unnecessary hospital admission. As part of our community nursing team we empower our staff to work autonomously and withflexibilityby providing them with state of the artnew tabletsfor them to be able to check in from home, write their notes up on the go and allow them to not be trapped in an office base, so they can really enjoy working in their chosen community background! We have a full-time role (37.5 hours per week) and a part-time role (30 hours per week) available. This role forms part of a fantastic team transferring to a new organisation under TUPE on 1 July 2026, and if successful, you will transfer with the service., Within this role, you will:
- Assist in providing nursing care as delegated by the Team Leader within patients homes.
- Maintain knowledge, skills and attitudes necessary to deliver a quality service that is responsive to the changing needs of patients.
- Provide clerical and administrative support to the team as delegated by the Senior Sister/Team Sister
- Deliver care in accordance with organisation policies, procedures and guidelines, according to competencies
- Report any changes in condition of the patient to the Team Leader or in their absence the designated nurse in charge of the caseload.
