Full Job Description

It's about helping people make lasting changes that improve their health, confidence and quality of life., We're looking for an enthusiastic and compassionate Health & Wellbeing Coach to join our multidisciplinary team at Arc Bucks Primary Care Network. You'll work alongside GPs, nurses, pharmacists, social prescribing link workers and care coordinators to support patients to take control of their health and achieve goals that matter to them.

Whether someone wants to lose weight, reduce their risk of diabetes, improve their mental wellbeing, become more active or better manage a long-term condition, you'll provide the coaching, encouragement and practical support to help them succeed.

You'll have the opportunity to build meaningful relationships with patients, deliver innovative health programmes and contribute to improving the health of our local communities.

You'll be trusted to coach rather than instruct.

You'll be encouraged to innovate rather than simply follow a script.

Most importantly, you'll see first-hand the difference your work makes to people's lives.

Purpose of the Role

To empower people to improve their health and wellbeing through personalised coaching, behaviour change and self-management support.

Working as part of the multidisciplinary team, you'll help patients develop the confidence, knowledge and skills to make sustainable lifestyle changes, improve long-term health outcomes and reduce health inequalities.
