Full Job Description

  • Contributing to antenatal and postnatal care programme
  • Completing HCP mandated reviews, packages of care and safeguarding.
  • Supporting in the supervision/ line management and development of colleagues
  • Undertaking a range of competent clinical and non-clinical interventions to support the outcomes for families and the service
  • Attend and contribute to team meetings
  • Building relationships with colleagues to enhance the team working environment
  • We have full and part time opportunities ranging from a minimum of 22.5 hours to a maximum of 37.5 hours per week between 9am to 5pm. This role is a community-based role so the majority of your duties will be carried out between your base and community settings or client homes., We know life outside work matters.     Kent Community Health NHS Foundation Trust is due to integrate with Medway Community Healthcare on 1 October 2026.
