Please note: at this stage, the role is open only to colleagues currently employed within Kent and Medway NHS Trusts, the Kent and Medway Integrated Care Board (ICB) and Medway Community Healthcare. As aCare Coordinator, you'll be a key point of contact for patients, carers, and professionals, ensuring joined-up support across health, social care, and voluntary services. You'll manage a caseload, coordinate care, and help patients access the right services to maintain their independence and wellbeing. In this rewarding role, you will:

  • Support housebound patients living with long-term conditions.
  • Help reduce avoidable hospital attendances and admissions.
  • Respond to referrals and coordinate access to services.
  • Provide advice and support to patients, carers, and colleagues.
  • Develop personalised plans that promote independence and prevent care crises.
