Full Job Description
Here for Health is a dynamic service supporting patients in the Waiting Well Pilot programme, helping them optimise their health before and after surgery. The team uses an evidence-based Lifestyle Medicine approach and behaviour change techniques to support sustainable improvements in diet, physical activity, sleep, mental wellbeing, social connections, and reducing harmful substance use. You will work across professional and organisational boundaries, collaborating with a multidisciplinary team and external partners to support patient outcomes and address health inequalities.,

- Data Collection & Service Evaluation
- Operational & Resource Management, Assess patients' health needs and deliver brief and extended health promotion and behaviour change interventions. Support self-management of lifestyle behaviours, including diet, physical activity, sleep, mental wellbeing, substance use, and chronic disease risk reduction, using motivational interviewing techniques.
- Programme Delivery & Coordination: Deliver patient-focused programmes, contribute to health promotion campaigns, and support service quality improvement projects. Develop and maintain referral pathways and collaborate with multidisciplinary teams and external partners to optimise patient outcomes.
- Stakeholder Engagement & Communication: Build and maintain internal and external partnerships. Communicate complex or sensitive information effectively, maintain confidentiality, and respond to mental health or safeguarding concerns in line with Trust protocols.
- Data Collection & Service Evaluation: Collect and analyse patient data to support service review, audit, and evaluation, monitoring outcomes and quality improvement.
- Operational & Resource Management: Contribute to service development, maintain professional competence, and ensure smooth operational delivery, including resource management and emergency response.
- Education & Training: Maintain professional knowledge and portfolio. Provide education and support to patients and contribute to teaching, supervision of students/volunteers, and team CPD sessions.
- Experience using motivational interviewing in professional practice
- Knowledge and skills in providing health promotion advice and support
- Ideally, stop smoking practitioner training

- First level degree (Level 5 or 6) in relevant subject
- Evidence of continued professional development Desirable criteria
- Motivational interviewing training
- Certified NCSCT smoking cessation practitioner
- Nationally recognised education/training in health behaviour change and healthy lifestyles including: - Brief Interventions - MECC - Alcohol & drug use - Sexual health, - Oral health, - Emotional well-being - Physical activity - Weight management, Experience of communicating health information and advice using a person centred approach.
- Experience in presenting information and delivering training.
- Experience in developing and maintaining information resources Desirable criteria
- Experience working in the NHS or a health care setting.
- Experience of working with colleagues from a diverse range of backgrounds.
- Experience undertaking basic health measurements such as blood pressure and BMI.
- Experience delivering health promotion campaigns.
- Experience of working across organisational boundaries to achieve a common goal.
- Previous experience of formal/informal teaching., Up-to-date knowledge in the speciality of healthy living and health behaviour change.
- Able to communicate complex and sensitive information simply and clearly, adapting style to suit the requirements of the audience.
- Willingness to learn and develop.
- Flexible, well-motivated, adaptable team member.
- Ability to work autonomously and proactively to support the needs of the service and initiate improvements. Desirable criteria
- Knowledge of Oxfordshire healthcare services.
