Social Prescribing empowers people to take greater control of their health and wellbeing. It addresses people's needs in a holistic way, recognising that health is determined by a wide range of social, economic, and environmental factors.

the new Health Inequalities Social Prescriber will empower patients to improve their health and wellbeing to take full advantage of screening programmes, in particular cervical screening, that save lives. Are you passionate about educating people on the benefits and importance of screening so as to live longer and healthier lives?

This exciting 2 years project role could be for you. You will be involved at a community level to support a 2 year project to make our borough healthier, are you up to this exciting challenge?
