Full Job Description

Facilitate access, to medical and other health and social/well-being related appointments for patients who typically experience barriers when accessing healthcare services.

Act as a focal point for signposting and communication between patients, doctors, nurses, other members of CAVHIS and external services.

Undertake various administration duties associated with patient care and the efficient running of CAVHIS., An exciting opportunity has arisen to be part of the Cardiff and Vale Health Inclusion Service (CAVHIS), which aims to deliver an equitable and high standard of care to vulnerable groups in Cardiff and the Vale. We are keen to recruit someone who shares our passion for this client group.

The successful applicant will have excellent communication skills and will work well within a team. The ability to cope in stressful and demanding situations is highly desirable, although support will be given.
