Health Care Support Worker

We have an exciting opportunity for a Band 3 Healthcare Support Worker to join our supportive and dedicated community team in Wandsworth. As a Healthcare Support Worker, you will provide high-quality care for patients with health and social care needs in their own homes and other community settings. Where appropriate, care will focus on supporting patients to maximise their independence and achieve their individual goals. Working under the guidance of a Registered Nurse or AHP and following an agreed care plan, you will deliver care tailored to each patient's needs. You will work closely with patients, families, carers and other members of the multidisciplinary team, reporting any concerns or changes in a patient's condition or abilities to ensure safe and effective care. This role involves working independently in the community, including lone working. You will receive support and supervision, with access to out-of-hours on-call support when needed. Working in partnership with patients, carers and colleagues, you will help ensure a person-centred approach to care that respects individual choices and preferences. You will treat all patients with dignity and respect and maintain confidentiality in line with policies and professional standards. Working Hours are Full time (37.5 hours per week) across the service hours of Monday to Sunday 07:30 to 23:00 Key Duties within this role include:

• To carry out pre-defined care plans with patients in their homes, as directed by a senior support worker or qualified therapist for a range of patients promoting an enablement ethos where appropriate, ensuring that patient's safety and care is prioritised at all times.
• Provide personal care support which may include washing, dressing, light meal preparation and light domestic duties.
• To undertake, and report back on delegated clinical activities that have previously been signed competent. For example vital signs and NEWS2, routine urinalysis, glucose monitoring, venepuncture, medication administration, gait/mobility training, falls prevention awareness groups, increasing independence through activities of daily living.
• To report back to the allocated Registered Nurse or Allied Health Professional, and document effectively care tasks undertaken, progress made, outcomes of intervention and any concerns regarding patients health, wellbeing or ability and to discuss when modification of care plans may be necessary as part of a structured clinical supervision system.
