Full Job Description
This is a great opportunity to build skills in children's community care while supporting families with complex health needs.
You'll be part of the Community Children's Nursing Team, working with nurses and other professionals to help children and young people stay well at home and in local settings. Your work will support safe, high-quality care for children with a wide range of needs, including those with long-term, acute or end-of-life conditions.
What you'll do:
+ Support the delivery of individual care plans for children and young people
+ Communicate with families and colleagues to share updates and concerns
+ Assist with clinical tasks such as phlebotomy, wound care and personal care as trained
+ Help run nurse-led clinics and maintain essential equipment and supplies
+ Keep accurate electronic records and follow all safeguarding and infection-control procedures
+ Level 3 in Health and Social Care or equivalent experience
+ Experience working with children, young people and families
+ Ability to plan workload, follow care plans and work safely under guidance
+ Good communication skills and confidence using basic IT systems
+ A willingness to learn new skills, adapt to changing needs and travel between sites
