Health Care Assistant NHS Health Checks

The NHS Health Checks programme is aimed at individuals aged 4074 who do not have a prior diagnosis of cardiovascular disease (CVD), diabetes, or chronic kidney disease (CKD). The role involves supporting the delivery of health checks that assess patients risk of developing these conditions and providing appropriate advice, interventions, and signposting to encourage positive behaviour change. The overall goal is to reduce the risk of premature death and long-term disability. The programme also promotes awareness of alcohol consumption in relation to recommended limits and includes raising awareness of dementia risk., By collaborating closely with GP Members, Patient Safety Partners and wider health and care organisations, combining shared insight and expertise to improve outcomes for local people.
