Full Job Description
To deliver NHS Health Checks to eligible Sutton patients within Enhanced Access clinics, undertaking clinical assessments, blood tests, accurate documentation, patient education, and followup in line with NHS Health Check guidance, Sutton PCN protocols, and NICE guidelines. Work in line with-
- NHS Health Check guidance
- NICE and SWL clinical pathways
- Sutton PCN policies and procedures, Deliver NHS Health Checks to eligible patients in line with the NHS health check programme
- Undertake facetoface assessments, including:
- Height, weight, BMI and waist circumference
- Blood pressure measurement
- Lifestyle assessment (smoking, alcohol, physical activity, diet, QRISK,QDiabetes)
- Blood test (e.g. cholesterol, HbA1c) Complete followup telephone appointments Accurately record all activity on the clinical system - EMIS Calculate and explain CVD risk scores in a clear and supportive way Provide brief advice and signposting to lifestyle services Escalate or refer patients appropriately in line with PCN protocols Ensure informed consent, confidentiality, and data quality standards are met Work across multiple practices as required by the PCN Adhere to infection prevention, safeguarding, and information governance requirements Deliver bloods to hospital at the end of each clinic. Participate in Health care assistant meetings to support the development. Identify abnormal results (e.g. raised blood pressure, cholesterol, HbA1c) and escalate appropriately in line with local protocols
Sutton has a population of approximately 200,000 residents registered to 21 practices and there are currently 4 Primary Care Networks (PCNs); Carshalton, Cheam & South Sutton, Central Sutton and Wallington PCN; each serving a population of approximately 50,000 patients. PCNs form a key building block of the NHS long-term plan. Bringing general practices together to work at scale has been a policy priority for some years for a range of reasons, including improving the ability of practices to recruit and retain staff; to manage financial and estates pressures; to provide a wider range of services to patients and to more easily integrate with the wider health and care system. (The Kings Fund, Primary Care Networks Explained, 1.7.19) Our Sutton PCNs are forward-looking, friendly and focused on providing a wide range of excellent healthcare services to patients in Sutton and the surrounding area. Our PCNs between them are led 9 PCN Clinical Directors. The PCNs work together as they see the benefits of working together in a larger GP partnership and are delighted to be realising some of those benefits now.
