Health Care Assistant

The post holder will work as a member of the Overnight Nursing Service providing direct and indirect patient care as directed by a trained member of the team in order to support patients and their families within their own home environment. The post holder will undertake clerical duties as directed including electronic documentation and working as an effective member of the Multidisciplinary team.,

• To provide basic nursing, therapy and social care to people and carers, carrying out delegated clinical, therapeutic and personal care tasks
• To promote the physical, mental and emotional health and wellbeing to individuals and client groups.
• To be an effective member of the multi-disciplinary team.
• Communicate effectively with patients and carers, recognising their needs for alternative methods of communication.
• To carry out care/other duties under direct supervision of a registered professional.
• To support the band 6 nurse with tasks in patients homes.
