Head of Beyond the Hospital Programme

Beyond the Hospital is Homerton's programme for redesigning care out of hospital and into people's homes and neighbourhoods. It brings together the redesign of the Trust's community services with the development of the integrated neighbourhood team model across the eight Neighbourhoods in City and Hackney, supporting the neighbourhood health commitments in the NHS 10 Year Plan and delivery of our 2026-2031 strategy, Our Future Together., The post holder will lead the design, planning and delivery of the Beyond the Hospital Programme across both workstreams. Key duties include:

• + Leading the redesign of Homerton's community services, from diagnostic and engagement through co-design to a costed implementation roadmap + Building and owning the case for change, using the evidence base to secure agreement across partners + Leading Homerton's contribution to the integrated neighbourhood team model with primary care, ELFT, LB Hackney, City of London Corporation and VCSE partners + Directing the external transformation partner, holding the contract to account for quality, pace and value + Holding delegated responsibility for the programme budget and reporting on spend and benefits + Establishing governance, including the Programme Board and reporting into Trust Leadership Team and the Place Based Partnership + Presenting highly complex, sensitive and contentious information, using negotiation and persuasion to overcome barriers to acceptance + Leading multi-year strategic planning and writing business cases for investment and service change + Line managing the programme team and supporting wider neighbourhood roles across the partnership + Ensuring patients and carers are involved throughout, in line with the City and Hackney co-production charter
  
  We are looking for an experienced transformation leader to design, plan and deliver this programme. You will bring significant experience of transforming community health and care, and the leadership skills to drive delivery through multiple partners, divisions and established teams.
