Full Job Description
An exciting new opportunity has arisen with the Hepatology department, HCC Surveillance Pathway Navigator is a new role to support patients identified as at risk of liver cancer to attend essential HCC surveillance scans and associated appointments with face to face and telephone interventions. We are looking for the right person to work proactively to identify and overcome barriers to engagement with HCC surveillance, particularly with patients experiencing inequalities in access to healthcare.
The new role will be working along side the Liver Team and Caner Alliance:
To maintain the HCC surveillance database for the trust.
To collect and submit the data required by the cancer alliance., To engage with patients undergoing HCC Surveillance due to their Liver Disease. Support scheduling of clinic and imaging appointments to ensure surveillance schedule is maintained. To coordinate the necessary appointments, or investigations to expedite patients with abnormal results (e.g. suspected HCC) and ensure the correct onward pathway is followed in a timely manner. Make telephone calls to patients to remind them of upcoming scans / laboratory tests; identify and assist in resolving any barriers to engagement; coordinate text reminders for surveillance appointments for patients. Support patients with additional needs to attend appointments - for example, meet at front door / attend appointment with patients.
Coordinate and provide administrative support to the clinical team to ensure all relevant paperwork and clinical information is available for consultations. Maintain and populate the patient tracking list ("Database") for the surveillance clinics and scans, adding newly referred patients. Document and monitor aspects of care coordination and service delivery for audit and reporting to the Hepatology team and the Humber and North Yorkshire Cancer Alliance. Respond in a timely manner to enquiries from the patient's GP regarding liver disease stage, care and treatment on the cancer surveillance pathway.
