GP Assistant- New Patient Health Checks

We have an exciting opportunity for a GP Assistant (GPA) to join our proactive care team with a primary focus on undertaking New Patient Health Checks. The post holder will support the delivery of high-quality patient care through a combination of clinical and administrative duties within their scope of practice. Working as part of the multidisciplinary team, the GP Assistant will undertake comprehensive New Patient Health Checks, collecting baseline health information, reviewing medical history and medications, carrying out routine observations, and identifying current and future health and care needs. The post holder will ensure patients are appropriately signposted, referred, and scheduled for any required long-term condition reviews, screening programmes, immunisations and follow-up care. The role will include health promotion, patient education, clinical coding, health records management and care coordination, helping to ensure patients receive timely, safe and patient-centred care. Working in accordance with the NHS England GP Assistant Competency Framework, the post holder will contribute to the ongoing delivery and improvement of primary care services. The successful candidate must be prepared to undertake training for the GP Assistant Certificate qualification
