GP Assistant- Diabetes

Lea Vale Medical Group have a new and exciting opportunity for a GP Assistant to support our patients with Diabetes to join our pro-active care team on a full time basis., We are seeking a proactive GP Assistant (GPA) to support the delivery of diabetes care within our practice. The post holder will play a key role in identifying patients due for diabetes reviews, coordinating follow-up care, undertaking routine clinical assessments and supporting patients to manage their condition. Working as part of the multidisciplinary team, they will contribute to the delivery of high-quality, patient-centred diabetes services through health promotion, patient education and care coordination.
