This modern practice offers a patient-focused service with 15-minute appointments and a capped number of 25 patient contacts per day. You will be supporting the safe, effective, and compliant delivery of primary care services, contributing to a forward-thinking, multidisciplinary team.,

• Deliver a mix of telephone, face-to-face, and home visit appointments
• Provide high-quality, evidence-based care to a diverse patient population
• Support patients with complex and long-term conditions
• Collaborate effectively with the wider MDT
  
  GMC-registered and on the GP Specialty Register
• Listed on the NHS Performers List
• Strong communication and organisational skills
