Conduct consultations, home visits (when necessary), and telephone/video consultations as required. Provide appropriate diagnosis, investigation, treatment, and referral in line with clinical policies and NICE guidance. Manage acute and chronic illnesses, promoting continuity of care and patient-centred decision-making. Provide clinical advice, support, and mentorship to the wider clinical and administrative team as required. Maintain accurate, comprehensive, and timely medical records using System One. Participate in the management of patients with long-term conditions, including proactive care planning. Undertake medication reviews, prescribing safely and effectively in line with practice and national protocols. Support and participate in local and national enhanced services and QOF activities. Team and Organisational Responsibilities Work collaboratively with the wider multidisciplinary team including nurses, pharmacists, ANPs, Paramedic and more. Attend and contribute to clinical meetings, significant event analyses, and quality improvement initiatives. Participate in the practices audit, governance, and safeguarding processes. Support the practice in achieving targets and service improvements in line with PCN and ICB objectives. Contribute to the education and supervision of trainees, students, or new staff as appropriate. Participate in practice development initiatives and contribute ideas for service improvement. Administrative Responsibilities Complete clinical correspondence, results, and documentation promptly. Support repeat prescribing systems and medication safety processes. Contribute to workflow optimisation and use of technology to improve efficiency. Professional Responsibilities Maintain registration with the GMC and perform within the scope of the NHS GP Contract. Engage in annual appraisal and revalidation processes. Participate in continuous professional development (CPD) and mandatory training. Adhere to the practices policies on confidentiality, data protection, equality, and health and safety. Actively contribute to a positive, inclusive, and supportive working environment.


• Full registration with the General Medical Council (GMC) and inclusion on the GP Register.
• Membership of the Royal College of General Practitioners (MRCGP) or equivalent.
• Eligible to work in UK general practice (NHS performers list registration).
• Experience in providing primary care services within a GP practice or similar environment.
• Experience in managing long-term conditions, acute presentations, and preventive care.
