Full Job Description
To support the Frail Elderly Assessment Team in providing excellent clinical care and effective management of patient assessments and discharges through a holistic approach to the individual needs of the patient. This includes liaison with Primary and Secondary multidisciplinary health and social care teams. To provide support to facilitate and drive the safe, effective and timely transfer of patient care from the acute to the most appropriate setting to meet the needs of the frail patient.Main duties of the job Support the Frail Elderly Assessment Team to deliver high quality assessments and discharges in keeping with the needs and wishes of patients and their families, acting as a patient advocate when necessary. To assist with a multi-disciplinary and multi-professional frailty approach to discharge planning and transfer of care, including the patient and carer in the provision of seamless care To assist the FEAT with provision of CGA to patients identified as having frailty. To help to coordinate the patient discharge needs and to assist in the development of a discharge plan for individual patients, including evaluating progress to ensure the pathway remains appropriate.
