Frailty INT Occupational Therapist

In this role, you will work collaboratively with multidisciplinary colleagues to deliver person-centred care for individuals living with frailty. You'll assess functional abilities, develop tailored interventions, and empower patients to achieve their goals in the community. Education / Qualifications and Relevant Experience Essential

• Professional Qualification -Degree or pre reg MSc in Occupational Therapy
• Current Professional registration - HCPC
• Broad range of post graduate experience of medical, surgical, orthopedic, and neurological settings
Desirable
• Has or is working towards a prescribing qualification
• Holds a masters level qualification
Skills and Aptitude Essential
• High level of specialist assessment and treatment skills.
• Is able to facilitate primary care activity, with a strong emphasis on prevention and early intervention, including through the delivery of public health advice (e.g. relating to physical activity, weight management and smoking Cessation)
Knowledge and abilities Essential
• Is able to plan, manage, monitor, advise and review general frailty care programmes for patients in core areas, including disease states/ long term conditions identified by local Needs Assessment
