We are seeking a dedicated Frailty Occupational Therapist to join our developing Three Harbours Integrated Neighbourhood Team in North Cornwall. In this role, you will work collaboratively with multidisciplinary colleagues to deliver person-centred care for individuals living with frailty. You'll assess functional abilities, develop tailored interventions, and empower patients to achieve their goals in the community. As part of Cornwall's integrated care approach, you'll help shape seamless working across health, social care, and voluntary sectors--ensuring the local population receives joined-up support at "place." Apply today and help transform integrated care in Cornwall!, Develop, implement and evaluate a seamless Frailty support service across theintegrated neighbourhood team, aimed at continuously improving standards ofpatient care and wider multidisciplinary team working.To support the elderly to age healthier, reducing progression of frailty and risksassociated with it where possible.To support them to make informed choices as to their health and welfare.To attend and contribute to MDTs and work collaboratively with neighbourhoodteams to support elderly and carers.Reduce emergency hospital admission, readmission and try to reduce care burden.To provide annual dementia health checks and home visits or face to faceappointments.Early identification and treatment of Delirium.To identify those likely to be within their last year of life and support them to plan fortheir future needs, To view a detailed job description and person specification including the main responsibilities of this role please see supporting documents. Education / Qualifications and Relevant Experience Essential

  • Professional Qualification -Degree or pre reg MSc in Occupational Therapy
  • Current Professional registration - HCPC
  • Broad range of post graduate experience of medical, surgical, orthopedic, and neurological settings
  • Desirable
  • Has or is working towards a prescribing qualification
  • Holds a masters level qualification
  • Skills and Aptitude Essential
  • High level of specialist assessment and treatment skills.
  • Is able to facilitate primary care activity, with a strong emphasis on prevention and early intervention, including through the delivery of public health advice (e.g. relating to physical activity, weight management and smoking Cessation)
  • Knowledge and abilities Essential
  • Is able to plan, manage, monitor, advise and review general frailty care programmes for patients in core areas, including disease states/ long term conditions identified by local Needs Assessment
