Full Job Description

The Urgent Community Response UCR and Frailty Hospital at Home team Virtual Ward provides a rapid assessment, diagnostic and treatment service to reduce unscheduled admissions for people with complex health needs, exacerbations of long term conditions and frailty.

UCR North delivers a joined up approach across the Basingstoke and Alton ICB locality to provide high quality urgent care and work closely with all GP practices, community teams and wider system partners.

The service runs from 0800 to 2000, seven days a week, 365 days a year.

The post holder will work closely with the Consultant Practitioner, Advanced Clinical Practitioners and GPs to support rapid delivery of treatment and avoid hospital admission where appropriate.

The post holder will provide clinical expertise by delivering assessment, advice and information in line with clinical guidance through telephone triage and face to face assessments.

They will carry out comprehensive assessment and care planning, including history taking, physical assessment and treatment planning without direct supervision. The role also includes complex case management using extended skills, where appropriately trained, to avoid hospital admission and support people with frailty and complex needs to remain safely at home., The post holder will act as a central contact for the UCR team, using initiative to triage phone calls and emails and ensure timely and appropriate action is taken. They will work closely with Triage Support, using judgement to delegate calls and documents safely. The role includes undertaking the deteriorating patient module at level 6 or 7. The post holder will manage telephone referrals from Ambulance crews, ED, 111, GPs, community teams and self referrals, using clinical reasoning to accept or redirect referrals. They will support daily handovers, attend One Team meetings and contribute to MDT discussions. The role requires working within scope of competence, using critical thinking, and carrying out autonomous assessment, history taking, physical examination and treatment planning. The post holder will manage patients with acute, chronic and complex needs and develop person centered care plans with patients and carers. They will proactively case manage using RIO, delegate tasks
appropriately, and deliver or direct holistic treatment programmes. Additional duties include promoting self care, supporting staff, ensuring effective clinical communication, maintaining accurate records, upholding confidentiality, mentoring students and taking part in regular clinical supervision.
