Field Care Supervisor

· Write and update new and existing care plans

· Coaching and mentoring care colleagues

· Attending care reviews

· Supporting the on-call service on a rota basis

· Providing "hands on" care and support

· Build and maintain positive relationships with our clients, family members, colleagues, and other professionals.

· Utilise technology to deliver quality care.

24 hours per week, with additional care cover when required.
