Full Job Description
We are looking for a dedicated and organised Field Care Supervisor to join our team at Home Support Matters. This is a key role supporting the delivery of high-quality, person-centred care within the community. You will be responsible for overseeing care delivery, supporting our care workers, and ensuring all services meet regulatory standards and internal quality expectations. Key Responsibilities Care & Quality Management
- Manage customer cases including assessments, care planning, reviews and audits
- Ensure care plans, risk assessments and MAR charts are accurate, compliant and up to date
- Conduct regular quality reviews, customer visits and satisfaction checks
- Monitor care delivery to ensure high standards in line with CQC and local authority requirements Staff Support & Supervision
- Carry out field supervision and face-to-face support for care staff
- Complete competency checks, including medication administration
- Support staff with performance concerns and risk assessments
- Provide hands-on care when required Compliance & Administration
- Maintain accurate records in line with company policy and GDPR
- Audit documentation including care logs and MAR charts
- Ensure contracts and service agreements match delivered care
- Complete incident reports and take appropriate follow-up action Partnership Working
- Liaise with health and social care professionals
- Support referrals (e.g. Occupational Therapy, continence services)
- Attend review meetings with customers, families, and professionals On-Call Support
- Participate in the on-call rota, supporting service delivery outside normal hours
Is passionate about delivering high-quality care - Works well as part of a team and independently
- Has strong organisational and problem-solving skills
- Communicates confidently with a range of people
- Is flexible and committed to continuous development Essential Requirements
- NVQ Level 2 (or equivalent), with willingness to work towards Level 3/4
- Minimum 2 years' experience as a community care worker
- Excellent communication and customer service skills
- Strong IT and administrative skills
- Good understanding of care standards and compliance
- Full UK driving licence and access to a vehicle Desirable
- Experience supervising or managing staff
