FEP / EIP Care Coordinator(Newly Qualified Social Worker / Nurse)

Are you a qualified Social Worker, or Mental Health Nurse with a passion for mental health care? We're looking for compassionate professionals to join our Early Intervention in Psychosis (EIP) Service - PATH (Psychosis: Prevention, Assessment and Treatment in Hertfordshire). Based in Stevenage, covering a caseload in the South East of the county, this is a rewarding opportunity to support people experiencing psychosis for the first time, delivering personalised and recovery-focused care. You'll hold a caseload and work closely with the wider MDT, helping you build strong relationships and develop your leadership and delegation skills. • an active member of a skilled multi-disciplinary team
• working together with your colleagues to provide our PATH service
• cover a designated locality - not the whole county
• travelling less and work out in the community, both in people's homes and our local hub buildings
• training as a family intervention therapist
You will be passionate in the delivery of recovery based and collaborative approaches to care planning, working flexibly and creatively with people in order to achieve their individual goals.


• Education to degree level or Academic qualifications equivalent (Diploma/Degree in in Mental Health Nursing /Social Work / Occupational Therapy)
• Registration with NMC/ HCPC/SWE
Desirable criteria
• Advance Training in mental health
• Approved Mental Health Professional/Best Interest Assessment training, At least 2 years of postregistration experience with at least one year at Band 5 (marginally less experience will be considered if applicant can demonstrate competence).
• Experience of working in acute mental health inpatient/day care services
• Experience of working in situations of crisis and emotionally demanding environments
• Experience and knowledge of safeguarding process; dealing with family breakdown; serious mental ill health and risk of selfharm.
• Experience of partnership working with service users and carers, demonstrating dignity and respect

• Excellent ability to communicate with a wide range of stakeholders including service users and carers, other professional colleagues, members of other organisations and local communities, verbally and in writing
• Experience of using electronic recording systems and ability to produce accurate, appropriate and timely reports., Car driver essential, or access to a car (unless you have a disability as defined by the Equality Act 2010 which prevents you from driving).
• Full UK driving licence
• Business Insurance to use car for work purposes, You must have appropriate UK professional registration.
