We encourage early applications to ensure consideration for a post., As a family support worker you will be supporting children and young people to reach their full potential and ensure they have equal opportunities, choices and rights within their community.
You will work alongside colleagues in assessing, planning and supporting care for children and young people up to the age of 18 years, with disabilities and their families.
You will manage your schedule and prioritise your work.
You will contribute to the development, planning and delivery of the Children's Disability Service, ensuring integrated working across service.
You will offer health promotion and information and advice to children and young people, professionals, families, carers and the wider community.
Interviews to be held on 27/4/2026, Work as part the children's community team delivering care to children/young people with care needs, and their families, in their own
home, or residential setting, or place of education with support and appropriate supervision which can be remote or direct, provided by a
registered healthcare professional.
Undertake personal care activities and clinical care duties with minimal support and supervision as delegated by a registered healthcare professional in accordance with the HEIW All Wales Guidelines for delegation.
Ensures the registered healthcare professional is regularly updated on the patients/client's condition and progress including functional
ability.
Promote and maintains a healthy, safe and secure environment for babies, children and young people, and their families.
Ensures the working environment is safe and hygienic following infection prevention and control policies and processes.
Contributes to the teaching and supervision of students and junior staff
Delivers role in accordance with Health Board/Trust Values and Code of Conduct for HCSW's in Wales., The Health Board is committed to supporting its staff to fully embrace the need for bilingualism thereby enhancing patient and service user experiences. In our commitment to increase the number of staff who are able to communicate in Welsh with patients and professionals, we welcome applications from Welsh speakers.
The ability to communicate in Welsh is desirable for this post. If you do not meet the Welsh Language requirements specified, the Health Board offers a variety of learning options and staff support to help you meet these minimal desirable requirements during the course of your employment with us.

Hywel Dda University Health Board plans and provides NHS healthcare services for people living in Carmarthenshire, Ceredigion, Pembrokeshire, and bordering counties.
We have over 13,000 staff and together we provide primary, community, in-hospital, mental health and learning disabilities services.
We work in partnership with the three local authorities, as well as public, private and third sector colleagues, including our valued team of volunteers.

+ Four main hospitals: Bronglais Hospital in Aberystwyth; Glangwili Hospital in Carmarthen; Prince Philip Hospital in Llanelli; and Withybush Hospital in Haverfordwest
