Full Job Description

This is an exciting opportunity for Family Practitioners within the 0-25 Together Service to work closely with disabled children in partnership with their families and alongside education and health professionals. There are opportunities within the Assessment Team, Locality Teams or the Short Breaks Review Team where you will work with a variety of children and young people. You will work autonomously and will be responsible for a caseload of children and families. You will support social work professionals by delivering Family Help, listening to families, recording their needs and considering ways in which local resources might help support improvements to their situations. Your primary focus will be to make sure that disabled children are included and achieving their full potential. At times you will support safeguarding activities and will need to be able to demonstrate an understanding of safeguarding approaches for children. Your work will be overseen by a social work professional and you should expect regular, skilled professional supervision.

  • Experience of working directly with disabled children and their families.
  • Knowledge of the role that social care might play in the lives of disabled children and their families.
  • Understanding of policy relating to children and young people with SEND.
  • Ability to interview people, record and present information effectively through discussion and in written reports.
  • Desirable
  • Ability to work with complex situations and conflict.
  • Ability to assess and record eligible and non-eligible needs using evidenced based practice.
  • Knowledge of all aspects of working with disabled children and young people and different stages in their development.
  • Experience in multi-agency working.

Driving required You'll need to have the ability to drive and/or travel around Hertfordshire, either using your own mode of transport or a company car to travel to other locations. English Fluency The ability to converse at ease with members of the public and provide advice in accurate spoken English is essential for the post (for those whose language is a signed language the provision of a sign language interpreter who speaks English to the necessary standard of fluency will be required).
