Full Job Description
- Undertake Early Help Assessments and coordinate support plans, providing essential targeted interventions for families in need.
- Act as a Lead Professional, ensuring that multi-agency plans are effectively implemented, monitored, and reviewed.
- Cultivate and maintain positive relationships with families, actively promoting engagement and supporting sustainable change.
- Coordinate and chair Team Around the Family (TAF) meetings, ensuring meaningful contributions from all stakeholders towards achieving agreed outcomes.
A Level 4 qualification or above in Social Care, Youth Work, Family Support, Education, Health, or a related discipline is essential. - A relevant professional qualification in family support, childcare, youth work, or community services is desirable.
- Evidence of continuing professional development is essential. Experience
- Proven experience working with children, young people, and families within family support, early help, education, social care, or community services is required.
- Demonstrated experience in coordinating multi-agency interventions and managing complex family situations is beneficial.
