Full Job Description

The post holder will be required to develop and run the Fracture Liaison Service (FLS) with the other members of the Falls and Bone Health Service (FBHS), which includes:

· Case-finding, across all sectors, assessing, performing diagnostic evaluations.

· Making recommendations for secondary prevention of osteoporotic fractures.

· Working with the other member of the falls services to ensure falls assessments are performed.

· Providing input into the Falls Service as appropriate in line with agreed guidelines.

The post holder is an autonomous practitioner and clinical lead, providing specialist falls and bone health assessment, intervention and advice for Older People in Croydon who have fallen or are at risk of falling.

1 To co-ordinate and be responsible for the development, monitoring and maintaining of the Fracture Liaison Service in line with the overall Falls and Bone Health service

2 To be an autonomous practitioner and to provide comprehensive and highly specialised assessment of falls and bone health in different clinical settings, including community based and hospital based clinics, GP practices, patients home.

3 To play a role identifying cases of patients in the community who require falls and bone health assessments, from various sources including Fracture clinics, GP practice and other community services.

4 To be an integral part of falls team, and to work closely with the Consultant and different members of the Falls team.
