Equity Support Worker

At Disability Solutions West Midlands, we're looking for an enthusiastic and committed Equity Support Worker to join our award-winning team and help deliver our Life Without Limits project, supporting young people with disabilities aged 16-25 across Stoke-on-Trent as they prepare for independent living.

What You'll Do

Working directly with young people aged 16-25, you will:

Deliver person-centred enablement support

Build positive, trusting relationships with young people

Undertake assessments and co-produced action planning

Provide one-to-one mentoring and practical guidance

Facilitate informed decision-making and independence

Help deliver engaging workshops and group activities

Support transitions from family homes or local authority care into independent living
