Enhanced Clinical Practitioner (Community)

We have a fantastic opportunity for an Enhanced Clinical Practitioner (ECP) or Trainee Enhanced Practitioner to work in the Derby City Community Service, working up to 37.5 hours per week.

We will positively consider applicants who fulfil the Person Specification, with the exception of non-medical prescribing and advanced clinical decision making/patient examination skills.

However, we will consider those applicants who do not meet this criteria and are willing to accept a Band 6 Trainee Enhanced Practitioner role until successful completion of both programmes and the completion of a training portfolio.

The role will be either Band 6 or 7 (dependent on experience and qualifications).

Enhanced Clinical Practitioner (Community)

The post holder will provide medical assessment and management and deliver the clinical function for the Home Visiting Service and Complex Care Team.

Where required the post holder will provide clinical support for the multi-disciplinary and multi-agency service to deliver effective, efficient, and high-quality services for service users, providing mentorship, guidance and support where needed to aid clinical decision making.

Alongside this the post holder will work with team members developing the necessary skills and competencies to meet the service need.

Working in conjunction with the Service Clinical Lead GP and Operational Manager the post holder will deliver a safe and effective service, developing new ways of working and clinical pathways in accordance with key local and national clinical standards, for the service areas of Home visiting, urgent community response, enhanced health in care homes and anticipatory care.

Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.

We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people's own homes and via virtual consultations.
