Are you passionate about helping people regain their independence and achieve the things that matter to them? Were looking for an Enabler to join our Intermediate Care and Reablement Service, working as part of a multidisciplinary team to support people within the community. Rather than simply doing things for people, reablement is about working with them. Youll encourage, motivate and support service users to rebuild their confidence and abilities, following individual care and reablement plans designed to help them become as independent as possible. No two visits will be exactly the same. You may be helping someone safely mobilize around their home, supporting them with washing and dressing, encouraging them with everyday activities, assisting with their dietary needs or supporting them to work towards rehabilitation goals set by the therapy team. Youll build positive relationships with service users and their families while working closely with therapists, clinicians, social care
colleagues and other members of the multidisciplinary team., The Enabler role at HCRG Care Group involves supporting service users to regain independence post-illness or injury.
