Full Job Description
This post will be supervising a team of enablement workers, creating and reviewing client care plans, supporting the scheduler in rota cover out with the schedulers working hours and supporting the officer with the day to day running of the service. This post will also involve the successful applicant providing assistance or support to people in their own homes. The tasks required will maintain, maximise and promote the independence and lifestyle of vulnerable people in order to avoid hospital admission and facilitate a supported hospital discharge.
Applicants should be able to demonstrate a self-motivated and dynamic approach. You should have good team working abilities, ability to communicate, supervise and effectively support your team with day to day care queries. Work as part of a multi-disciplinary team to deliver personalised care whilst ensuring health and social care standards are maintained along with NHS values. Applicants should ideally have an SVQ 3 or above. A driving licence as well as use of your own car are essential for this role. Generous fuel allowance paid.

- Effective from 1st April 2026, the Agenda for Change full-time working week will be reduced from 37 to 36 hours. Part-time hours will be adjusted on a pro-rata basis. There will be an increase in the hourly rate to ensure that pay remains unaffected.
