Full Job Description

Deliver personalised care plans including personal care, medication, domestic tasks, and wellbeing support.
Collaborate with a multi-professional team and contribute to multi-disciplinary discussions.
Accurately record progress and escalate concerns when needed.
Receive ongoing support and training to help you excel and grow in your career.

A caring and compassionate personality.
A commitment to treating people with dignity and respect.
Creativity, forward-thinking, and the ability to work independently.
Ability to use smartphones/apps for record-keeping.
A valid driving licence and access to a vehicle for work (essential)
Willingness to work on a rota including unsocial hours.
