Are you a flexible, empathetic and collaborative individual with experience of providing support, advice, advocacy and communicating effectively the needs of clients to other professionals? Do you have experience of supporting vulnerable young people and adults who have experienced or are at risk of violence or significant harm? Will you exemplify our PROUD values? Patient First, Respectful, Ownership, Unity and Deliver If so, Sheffield Teaching Hospital is looking for an Emergency Department Support Navigator to join our team at the Northern General Hospital and provide vital support for those young people and adults who attend ED or who are admitted to the hospital right through to their safe discharge back into the community and ensuring that they have access to appropriate longer-term support. Support to clients while they are in the hospital and also in the community post discharge. Our successful candidate will identify and assess victims of violence, or those at risk of violence,
and provide a comprehensive and holistic assessment, advice, referral and support service, exploring innovative and effective ways of supporting them which will reduce their risk of re-victimisation., Provide vital support for those young people and adults who attend ED or who are admitted to the hospital right through to their safe discharge back into the community and ensuring that they have access to appropriate longer-term support.
