CLCH Hertfordshire Adult Community Services - CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting! The purpose of the EIV pathway is to deliver crisis management to patients living in Herts Valleys and to prevent a hospital admission, providing a clinical assessment by a nurse or paramedic jointly with a HCA. The operational hours of this pathway are currently 08:00 - 20.00 - 7 days a week (subject to change once fully established). The EIV pathway is closely aligned to the Unplanned Care service, which delivers high-quality clinical care to patients in their own home at times of crisis. The purpose of the Rapid Response (RR) pathway is to prevent hospital attendance/admission, and the Discharge to Assess (DTA) pathway is to facilitate early discharge from the hospital. The Unplanned Care team operates from 08:00 to 22:00, 7 days a week. The newly established EIV will supplement and support the delivery of Rapid Response and Discharge to Assess in the Unplanned care service.

  • Provide skilled, effective and evidence-based clinical care to patients in a variety of community settings.
  • Enable emergency provision of equipment through effective prescribing and ordering.
  • Make risk assessments, establish the safety or otherwise of patients ability to stay at home, and arrange necessary nursing and supportive packages.
  • Identify safeguarding concerns and effectively manage concerns or refers appropriately for safeguarding investigation. Apply knowledge of safeguarding processes and policy.
  • Provide autonomous clinical care to a range of patients presenting with undifferentiated, undiagnosed and/ or complex health needs and long-term conditions.
  • Assess and manage risk, knowing how to intervene proportionately and ensuring people are protected from harm, while protecting their human rights. Apply knowledge of the Mental Capacity Act and Care Act.
  • Provide evidence-based clinical/therapeutic interventions based on 'best practice', in order to improve health outcomes and promote choice.
  • Work in close collaboration with all other community agencies, including Locality planned care teams, In-Patient services and GPs.
  • Deliver agreed clinical quality and performance standards within the Unplanned Care team.
