Early Intervention Team Care Coordinator

The Bury Early Intervention Team for Psychosis is delighted to be able to offer a Secondment post of a full time band 5 Care Co-ordinator. Registered Mental Health Nurses, Social workers and Occupational Therapists are welcome to apply. The Bury Early Intervention Team supports those patients experiencing First Episode Psychosis and those whom are at risk of developing Psychosis. We are based within 3 Knowsley Place, which sits alongside hubs for social care and Community Mental Health Services. You will be expected to travel the Bury Footprint as part of the role and contribute to the duty rota. We would support applicants wishing to apply who are completing a preceptorship. You will work alongside passionate and experienced colleagues and be supported by the Team Manager and Clinical Lead. The post is a full time position (37.5 hours per week) and the services core hours are Monday to Friday 9am-5pm. Overseeing a caseload of service users with first episode Psychosis aged 14-65 years. Participation in a duty rota. Support clients in their recovery, offering Psychosocial interventions. Care planning to support recovery focused goals and interventions. Participate in offer carer support and education and family intervention. Support health promotion and work with other agencies/MDT. Signpost and refer. Support culture of learning, supporting students nurses. Adhere to safeguarding policies and legislation. Undertake Mental Health Assessments and reviews and comprehensive risk assessments and management plans. Medication management and administration (registered nurses).
