Full Job Description
You will work extremely closely with our Advanced Clinical Practitioner Nurse Specialists and assist, support and co-ordinate a smooth patient journey and pathway. A large part of the role is providing administrative and co-ordination support to patients, families, consultants, cancer nurse specialists and other staff involved in the early part of the cancer pathway. The successful applicant will ensure the efficient channelling and close co-ordination of the outpatient pathway ensuring robust communication between medical staff, GPs, patients, relatives, members of the public and other disciplines within the Trust., An essential part of the role will be the working under the guidance of the clinical teams to co-ordinate patient appointments and diagnostic testing, also collection of data relating to the cancer patient's journey. Other key responsibilities of this role will be understanding the importance of data quality and managing a varied workload. This varied workload will include patient contact via telephone, being a point of contact for referrals and service queries, typing letters where required, organising patient appointments and supporting the management of the MDT. The successful applicant will ensure the efficient channelling and close co-ordination of the patient pathway ensuring robust communication between medical staff, GPs, patients, relatives, members of the public and other disciplines within the Trust.
We are looking for an enthusiastic and motivated person to fulfil the post of Support Worker (Early Diagnosis Navigator), The post holder will demonstrate an understanding of cancer services, have excellent organisational skills, must be flexible in approach, able to exercise initiative and demonstrate a consistently high standard of professionalism, being aware of the need for confidentiality and integrity. The post holder will have excellent communication skills and be willing to undertake ongoing training and development. In addition, the post holder will be able to signpost patients and carers to the relevant sources of support and information appropriate to their needs.,
- Educated to Diploma level and/or equivalent level of experience and/or equivalent qualification
- GCSE English and Mathematics or equivalent
- Evidence of continued professional development, Essential criteria
- Experience of working within the NHS (either within primary or secondary care) Desirable criteria
- Knowledge of relevant cancer treatments, interventions and terminology
- Understanding of Macmillan Cancer Support and its role across the UK, Efficient in the use of Microsoft Word for Windows, Excel, Power Point, Outlook and internet. Desirable criteria
- Knowledge of medical terminology
- Knowledge of personalised care, Demonstrate excellent communication skills both verbally and non-verbally with people at all levels
- Able to prioritise workload
- Practical problems solving skills
- Ability to retrieve information from a wide range of sources and in different formats
- Tactful and diplomatic with the ability to ask sensitively about information needs.
- Able to use own initiative and work without supervision, Must be prepared to work across different hospital sites when required
