Full Job Description
An exciting opportunity has arisen for a District Nurse Team Leader to join the Community Nursing Team based at South Westminster Centre for Health. This is a key clinical and operational leadership role within community services, responsible for leading a designated community nursing team and ensuring the delivery of high-quality, evidence-based care to patients within their own homes and other community settings. The post holder will provide visible leadership, promote effective multidisciplinary working and support the delivery of safe, responsive and person-centred services. Working closely with the Service Manager, Clinical Lead and wider system partners, you will contribute to the delivery of integrated care pathways, support service improvement and ensure care is delivered in line with local priorities and commissioner requirements. This role offers an excellent opportunity for a registered nurse with community experience who is committed to improving patient outcomes, promoting independence and delivering high standards of nursing care within the community., As a District Nurse Team Leader, you will:
- Lead and manage a designated community nursing team, ensuring the delivery of high-quality, evidence-based care.
- Oversee the day-to-day operational management of the team to support safe, effective and responsive services.
- Promote effective multidisciplinary working within the wider Primary Health Care Team, delegating activities appropriately to support patient care.
- Manage team and individual performance, ensuring professional standards and development needs are met.
- Support the Service Manager and Clinical Lead to deliver high-quality, cost-effective services that meet local priorities and commissioner requirements.
- Manage a complex caseload, providing proactive case management to support patients with complex needs and prevent avoidable hospital admissions.Undertake holistic assessments of patients' physical, psychological and social care needs, including the needs of carers.
- Act as an autonomous practitioner, advocating for patients, promoting independence and supporting self-management where appropriate.
- Work collaboratively with primary care, acute services, social services and partner agencies to ensure seamless, integrated care.
- Provide information, advice and support to patients, relatives and carers to promote positive health outcomes and wellbeing.
