District Nurse Team Lead

As part of our move to neighbourhood-based working, you will play a key role in leading integrated care delivery alongside primary care, social care and community partners, ensuring patients receive high-quality care closer to home. We are looking for an experienced Band 6 District Nurse ready to take the next step, or an existing Band 7 leader seeking a new challenge. You will support patients with frailty, long-term conditions, complex healthcare needs and end-of-life care requirements while helping shape the future of community nursing services.   Hold a Specialist Practitioner Qualification in District Nursing or have equivalent senior community nursing experience.
